This scenario was my first exposure to the terrifying world of Fear Itself. Invasive Procedures is often touted as one of the scariest and most brutal scenarios ever written for TTRPGs, and one can immediately see why from its subject matter. Hanrahan explicitly wears the inspiration for Invasive Procedures on his sleeve, attributing it to the Silent Hill franchise and Hellraiser (1987). The way that the scenario written is so effective in pacing and content that it even kept me awake at night. Let’s just say I had one eye open on my door, just in case any Night Nurses decided it was time for me to undergo experimental surgery!

Game Masters should be aware of some mature themes in this scenario. There is a lot of body horror, almost all of which is described in grisly detail. The scenario also plays on the theme of imbalanced power dynamics between doctor and patient, and how a position of power may deter the righteous from exploring issues any further. If a player has had a negative and/or traumatic experience in a hospital, it is recommended to steer clear of this scenario unless discussed prior.
